• Course Changes Form

    This form is used to request changes to online and hybrid courses that have already been created in Banner. It allows the eLearning & Instructional Innovation Center to track, review, and process modification requests efficiently. Due to the time and coordination required to complete these updates, all requests must be submitted through this form. Please provide detailed and accurate information to help ensure timely processing. Please keep in mind that the database of CRNs used in this form is updated at the end of the day everyday.
  • Please note that some changes can only be made if there are no students enrolled in the course.
